Moving on Together North East
Moving on Together is a lottery funded service that geographically covers the North East of England.
Funded from January 2016, the service delivered by Momentum, Stroke Association and Brainbox aims to make up to 4 keys differences to the lives of children / adults with an Acquired Brain Injury and / or Neurological Condition(s) and their families / carers. It will reduce beneficiaries' social isolation through goal planning activities focusing clients accessing their local community with support of staff and volunteers.
It will improve beneficiaries’ physical and psychological wellbeing through the development of peer group activities, giving beneficiaries the knowledge, skills and confidence to self-manage their conditions and offer access to health related activities including the gym, walking and peer support.
The project will support beneficiaries as a family unit to prepare for, and manage the transition process from child to adult services. There are often unrealistic expectations associated with this process which will be managed using the ‘ready, steady, go’ transitional tool. The project will finally increase the work readiness of beneficiaries by providing ‘job clubs’ offering support to prepare for employment. The project will engage with employers, facilitating work placement and volunteering opportunities for beneficiaries to gain confidence and reduce the anxiety associated with returning to work after and ABI or diagnosis of NC.
Each individual referred to the project will complete an Initial Assessment which will formulate their individual SMART goal plan outlining goals and responsibilities of each organisation. From this point, the project will deliver support to the beneficiaries through 4 main activities:
• One to one support (including psychological support, counselling, travel training)
• Group based activity
• Transitional Activities
• SMART Goal Planning
This service is currently funded via the Big Lottery therefore no charges are recouped via service users or referrers.
