South of Tyne Community Integration
Momentum Skill’s South of Tyne Community Integration programme offers services to adults with neurological conditions and acquired brain injury in South Shields, Sunderland and Gateshead.
The service aims to reduce social isolation by providing support to access activities within the local area, according to client needs and interests. We can utilise existing provision where appropriate or set up groups where there is a need.
Activities are based on interest – such as photography, singing, art, or sport - or based on condition or circumstances – such as epilepsy, brain injury, carers, female/male only support.
The service is person-centered and can work with people on a one to one basis as well as in groups and is tailored to their individual circumstances. For example the service can provide adjustment sessions, cognitive or travel training.
The aim of the service is to promote independence and all activities will reflect this - gradually building engagement in community activities, then phasing out our support, working with the client to ensure this is at the right pace.
